Associate Responsibility & Eligibility Notice:
It is the responsibility of each enrolled associate to notify ASP Benefits in writing via email of any employment or status change, including retirement, reduction in eligible work hours, or termination. Failure to provide proper notification may impact benefit eligibility, and premiums will not be refunded for any period in which the associate was no longer eligible but did not notify ASP Benefits as required.
