Enrollment & Coverage Details
Guaranteed Issue Enrollment
Enroll within 30 days of becoming eligible or during Annual Open Enrollment to receive Guaranteed Issue Life Insurance—with no medical questions or underwriting required.
Eligibility Requirements
To enroll, associates must:
- Actively work a minimum of 30 hours per week
- Meet the minimum annual income requirement for the selected coverage amount
- Enroll in associate Life Insurance before adding eligible spouse or dependent child coverage
Maintaining Your Coverage
To keep your coverage active, notify ASP Benefits in writing via email of any employment or eligibility changes, including:
- Retirement
- Reduction in eligible work hours
- Termination of employment
- Any other change that may affect your eligibility
Failure to provide timely notification may affect your eligibility. Premiums cannot be refunded for periods during which coverage remained active after eligibility ended.
Policy Highlights
Coverage terminates upon retirement.
New associates ages 65–69 are eligible to enroll in $50,000 of Life Insurance coverage only. Current associates age 65 and older should contact ASP Benefits for available coverage options and premium information.
Personal Life Insurance and Accidental Death & Dismemberment (AD&D) benefits are reduced according to the master policy:
- Age 65 – 35%
- Age 70 – Additional 25%
- Age 75 – Additional 15%
- Age 80 – Additional 15%
Eligible spouses may enroll for up to $25,000 of Guaranteed Issue Life Insurance. Spouse premiums are based on the associate’s age. Spouse Life Insurance and AD&D benefits reduce by 35% when the associate reaches age 65 and terminate when the associate reaches age 70 or retires, whichever occurs first.
Dependent Child Life Insurance includes:
$250 for children ages 14 days through 6 months
$10,000 for children ages 6 months through age 19 (or age 25 if a full-time student)
